To discuss:
  • Associations between the prescription anticholinergics and development of dementia have been recognised for many years but little action has occurred because of uncertainty as to whether (certain) anticholinergics cause dementia. Applying the Bradford-Hill criteria for causation to the evidence base, there is now compelling evidence of a causal link, particularly for oxybutynin.
  • Pharmacists have a key role in identifying anticholinergic prescribing in older people, and the use of anticholinergic burden scales can be helpful in doing this.

  • Now is the time to act on reducing the risks of dementia from anticholinergics, and pharmacists have a crucial role in educating prescribers, working with other clinicians to address anticholinergic burden, and influencing policymakers at local and national level
